Health Plus is a service provided in conjunction with the Primary Care Health Trust. It enables us to offer a service out and about in the community and recognises the links between poor housing and health.
CHAS Health Plus Advisor can assist you with:
Benefits advice - Disability Living Allowance, Attendance Allowance,Carer's Allowance, Income Support, Jobs Seekers Allowance and Employment & Support Allowance.
Tax Credits
Debt advice - mainly issues with utility bills (gas, electric, water, etc)
Advice for people who would like to be rehoused
Applications for disabled bus passes and blue badges
Occupational Therapy assessments
Help with visa and passport applications

Monday mornings - Westbourne Green (patients of Drs Qureshi and Sai)
Monday afternoons - Manningham Medical Practice
Tuesday mornings - Barkerend Health Centre
Tuesday afternoons - Barkerend Health Centre
